Please bring the above forms completed, a list of your current medications and your insurance card(s) to your first visit. If you have a co-pay, please be prepared to pay at the time of each visit. Please allow 24-48 hours for prescriptions to be called or faxed to your pharmacy. We hope that your experience at Premier Health will be as pleasurable for you as we know it will be for us.
